Efficient well written code, responsive user form to collect and store into user MySQL database (html5, works on browsers, iPhones, Android, tablets)

- Email, Password
- Firstname, Lastname, Street address (address street, city, state, zip, etc.)
- Country pulldown list (US default)
- Date of Birth (optional) (month pulldown, DD, YYYY (also optional))
- Phone (optional) prefix pulldown like +1 etc... default from country choice

- Retrieve and save Latitude & Longitude from street address - Google
- Retrieve and save Timezone from address - Google
