This contest is to redesign the first two pages of the UCC-CL assessment, not a logo design contest.

We sell a product called the Underlying Characteristics Checklist (UCC) - it is a non-standardized assessment tool intended for use with individuals of all ages who have been diagnosed with autism spectrum disorder or who exhibit behaviors that are often related to autism and require intervention. Teachers, counselors and therapists fill out the assessment based on their experience with an individual who is displaying characteristics of autism.

It has been brought to our attention that our product is not up to the standards of other similar assessments. Other assessments are more professionally laid out and we would like to have something comparable.

We have 5 different assessments in this series created for for different situations but all look the same, and all 5 will need to be redone, so the winner of this contest will also be hired to layout all 5 assessments which are approximately 16 pages each.

This product is printed in an 8.5 x 11 booklet and sold in sets of 20 of the same type of assessments. Please make your design GRAYSCALE, not color, and include the AAPC Logo on it

I have included two examples of other assessments that were provided to me as examples of assessments that were better designed than ours. Please redesign the first two pages of the UCC-CL assessment to look similar to these examples, but different enough that it can be considered an original design.
