Small access2000 database and admin area for data capture form

We require a data capture form in .asp that captures visitor’s details (fields below). We require this to send the data to a database developed by yourself to show the details in an administration area (.asp), password protected, simple layout with the ability for the administrator to export the current database as a csv file.
The fields required for the database are as follows;
• firstname
• surname
• address
• postcode
• daytime_tel
• evening_tel
• age
• gender
• occupation
• message
The administration area should have the ability to edit and delete visitors profiles.

Htdocs (folder)

Confirmation of deliverables;

• very basic looking form2mail and sendmail .asp pages (for us to brand up and design) using the fields above
• Access2000 database to store captured information in, using the fields above
• administration area, password protected to display captured data, plus ability export the current database as a csv file. and to edit and delete details
• tested that data can be recorded in the database/ admin area through the form2mail pages, a csv file can be exported and details can be edited and deleted in the admin area
