Do some Excel Work

We are looking for someone who can do some data entry work for us. We would be sending you some scanned documents and you will have to enter the data provided in the scanned document into an excel sheet. All the data that you have to enter will be in the scanned document. You have to go through each document and enter the data in an organized format. Please go through the attached sheet to understand what all fields we want in the excel sheet. All the data would be provided to you.
