Create Drupal 7 entityform page, and some local task and other menu items

I need to have an entityform created which will store submissions with file attachments and email administrators. There will need to be two menu items and a link created. One menu item in the navigation menu and one in the main menu. These are easy. A link should appear at the top of an add node form page as a "local task" menu item. This should say "Click here to add a songbook or compilation" and link to the entityform page. Here are the form fields we want included in the form: 1. Title of songbook/audio collection (text) required 2. Source (text) 3. Category (text) required 4. Language (text) required 5. Mp3 files upload (zipfile upload) required 6. Songbook upload (PDF file upload) 7. Images upload (multiple images) 8. Commercial Links (text) 9. Translator (text) 10. Where to Get a Copy (text) I'm open to the form being implemented as a node form/content type. If you have a strong opinion about which way would be better, or the pros and cons of each method, just let me know.
