Breakdown Excel data

I need the following excel file broken down 1. Please match the columns in the excel to this docment 2. Then It's the list of all registered voters in the City of Syracuse. There are 19 Wards in the City of Syracuse, each with several election districts contained therein. We need excel spreadsheets for the Prime voters in all 19 Wards, by election district A prime voter is a person who has voted in 3 or the last 4 general elections or if a person has voted 4 times since 06 EG: GE12, GE10, GE09, GE08, or GE12, GE08, PP08, GE07 Ideally you would provide a mysql databse of all the data if possible
