Create and integrate on established website using Symphony CMS

AS per discussion with Shankar originally direct via email: Website is using Symphony CMS We would like to have a 2 page form created and integrated on our website www.currumbinrsl.com.au. The form would contain a set of questions that users would be able to answer online and submit. The question examples are attached (some with simple text input box some with multiple choice questions - no uploading of files is required). The questions may require us to change in the future - therefore we would prefer dynamic, however if this costs much more we would not do. An example of the form process can be seen in the attached (grilled). However we will only need 2 pages, will have less questions and have our own branding. We would require you to design the form with our branding in mind (similar to attached application form - but better) Our questions also attached (application form).
