Create an Excel File to Populate a Word Document

I would like someone to create a Button in Excel Document that Creates a Word document from the filled in Fields that are in Excel. The Excel file attached contains a list of all the fields and example data. I also need the attached Word document to be the template to populate. You will need to place the fields in the word document. Ideally I would like it to work with Excel 2003 and Word 2003 but if that is going to be a big problem then 2007 + would be OK.
