Company Letter Head Design - Quick Easy Job - URGENT

PLEASE READ THIS FIRST: I REQUIRE A SAMPLE OF THIS PROJECT WHEN YOU PM ME - PLEASE DO NOT PM ME WITHOUT A SAMPLE OR YOUR PM WILL BE IGNORED. THANK YOU. I currently have a logo designed for me which is attached in jpg and a vector based psd format. I need a company letter head with a professional looking header and footer. Ideally I would like the document to be in a .doc word format so that we can simply type letters and print them with a deksjet. IT IS IMPORTANT TO NOTE that my printer is a deskjet and therefore i cannot print to the edges so any designs must allow for a white border somehow. I would like to stick to a blue-ish theme that ties in with the logo. The details to record in the footer are in the text file attached. I need the work finished tonight, it is quite a quick job and is urgent. Please pm me samples if you want the job.
