Design a quote booklet for a holiday

Hi there, I'm looking for someone to design a quote booklet where I can just add in the custom details to each new customer. The quote document should have some standard information and then a section to be able to add unique information to each new customer. Example information that would need to be added in flight details, dive holiday details, dive boat details, extras avaiable etc- an editable PDF would maybe do the job. I already have graphics for the background and a logo which would need to be used., I would like to see some previous projects before awarding the project to someone
