Design a Website Mockup

I will provide you some basic requirements that you will be implementing over a small web page... Requirements: A web page having a title, heading, 3 text boxes, a checkbox, a submit button and a table with headers Textboxes: Name of a device (plain text) IP Address of a device (with dot separated input and validation) Number of radios in device (numeric validation) Check box: Should decide whether device is enabled or disabled (if disabled show entry disabled in table) Submit Button: On click of the submit button, the entered values should go into the table Table: It should have 4 columns NAME IP ADDRESS RADIOS TYPE (type should show a small image of the device, use an 64x64 image and same image in all the rows) Do not use any server side code, set all the data using JavaScript only. Try to give it a very clean and professional look to each element of the web page. Have a proper theme for all the elements of your page Do a proper commenting in all your code. Use your imagination to style your webpage in a different way Try to use features of HTML5, CSS3 and JavaScript libraries
