Design a Website Mockup

We are developing a new shopping path for purchasing health insurance online. Currently there are 5 pages in this process with varying graphic design needs. Some pages will just need to be made more consistent with any new styles, others, will need more design help through out. In general, we are not looking for a total re-branding, just clean-up and beautification and maybe some help addressing UX issues. Page headers and footers should be left alone at this time. Below is a description of the work request for each page in the shopping process: 1) home page - Page Description: Customer come here to enter some data and shop for plans. Work description: no work here is requested at this time. 2) Government Launch page Page Description: The customer comes to this page when they select a particular health plan on the home page by clicking the "enroll" button. The goal of this page is inform the user of what to expect when they reach the government website. The user must be redirected to the government site to verify their identity. Work Description: Not much work but to just apply any general conventions created with the new page styles. 3) Planmatcher Landing page Page Description: When the user completes filling out forms on the government website they will be directed back our website where the user must enter their name so we can retrieve their data from the government. Work Description: Not much work but to just apply any general conventions created with the new page styles. 4) Review Results Page Description: After we retrieve the users' data from the government we can display it to them on this page where they can review it for correctness, agree to terms, and sign. Work Description: This is one of the main the focus points of the this project. We just threw it together quickly and it looks ugly. A redesign is requested. 5) Change Plan (only used if user chooses to change plans from what the selected on the home page) Page Description: If the user chooses they can also change the health plan they selected by clicking the "change plan" button on the page above. This page should just show the user their current plan and allow the user to change plans. Work Description: This page was also just thrown together and needs a redesign. 6) Completion page Page Description: This page just confirms what the user is finished and set to be insured on some particular start date. Work Description: A redesign is requested.
