Design an iOS diet app Mockup

My app is pretty simple. It's a diet app, with static screens(no fancy dynamic effects). I need you to provide me the background images and style for labels/text fields/buttons.

I'll describe what i need.

1. I need the logo: with the option to change the text on the logo image. As text example try "Healthy Diet Calendar" (something with a calendar and the text above)

2. launch screen image: text("Healthy Diet Calendar") + small image + gradient background(or something similar) - again option to change the text

3. submit data screen: background + several labels, text fields, submit button style

4. profile screen: background + welcome name on the left + "submit new data" label on the right + big block with current day date and some extra text + several buttons fullscreen length (with some image on the left or something similar to not look bad)

5. calendar screen: background + style of label under calendar. No need to style the calendar

(for point 4 and 5 check this app [login to view URL] Their screens are almost the same as mine. As for colour scheme, mine would not be so girly. I want a combination between the colour scheme of the images you get when you search for "dukan diet" and "rina diet" on google)

6. list screen: background + 2 buttons on top + tableview(no need to style the table view, just mentioning it here to know it would take almost half of the screen)

How would you provide me the designs? As PSDs or iOS app with just the screens and no functionality? Can you also provide an estimation?
