Create Form on Wordpress - Repost - open to bidding

I need a form modified on a wordpress site within the next 2 hours. This project is extremely urgent. It should only take a few minutes to do. The link for the site is: http://www.reoconvention.com. Here's what we need: The layout is ok the way it is, except that the background cuts off halfway through the form. Also, the "register now" button is misplaced on the screen when viewing on a computer. It's correct when viewing on a mobile device though, so maybe there's a setting that's incorrect? Completed form needs to be correct on all devices. The only other adjustment is the "Guest Name" text box needs to be a little larger. That's it. Quick easy project. But it HAS to be done in 2 hours. No exceptions. PLEASE DON'T BID UNLESS YOU CAN ACCOMPLISH THIS. Thanks for your help!!!
