Build a Website Flow

Hello, I have a standard ASP.NET Web Application with a SQL Sever database. It is using the default project template for Visual C#, including account and identity management. When users register on the website, they enter a username, such as plavelle. I need the ability for other, non-logged in users to visit a page on the site, such as [login to view URL], and leave information about that user on a form. The non-logged in user should be prompted on the first page to enter their email address and check terms and conditions box, and then be taken to the form to enter information. Once submitted, the information should be stored in a SQL server database so it can be later queried on behalf of the user. The deliverable of this project will be 3 pages: Page 1: Validates that the username exists (plavelle). If so, presents the email and terms and conditions prompt to the user. Otherwise, shows an error that the user cannot be found. Page 2: Collects information on a form about the user. For now, it should be 1 multiline textbox and 1 rating from 1-10 (both required). When the user clicks submit, validation is done, the form information is stored along with the email of the submitter and the username included as part of the URL. If anything is invalid, a notification message should show on top of the page. Page 3: Thank you / Confirmation page after success I posted quick mockups of page 1 and page 2. Page 2 will also need a way to gather a 1-10 ranking. It is important that the implementation fit within the coding practices and standards of the default ASP.NET application. However, these pages should not share the Master Page with the rest of the application. Instead, a new master page should be created that can be skinned differently.
