Design templates for software - popups, embedable forms - open to bidding

Create a series of designs for popups and embeddable forms for my new software. Each design needs a headline a question area and a selection of different call to action buttons/lists/text areas. For each design we need: 1:optin form 2: choice buttons 3: drop down list 4: single line text area 5: multi line text area 6: check boxes I have actual designs from other designers, but i was not happy with them so wanted to find someone with more talent ;) We need a lot of these templates designed, ideally about 20-30. So the project price for each we would like to keep around $100 but we can negotiate dependant on quality.
