Analyze a small business PPC Campaign (healthcare)

This project is only about looking at my adwords campaign and giving me a realistic picture of what traffic increase, improved conversion rate, and cost reduction I can realistically expect if you would optimize it. A follow up project will involve working on the actual campaign(s).
Goal of my campaign is to increase patient numbers of my medical practice, which is a start up business.
My challenges:
I have an adwords campaign in place, that brings some traffic, but it is pretty expensive.
I target visitors from the close local area only; people in my area do not like to travel far.
The keywords where I need to be ranked well are highly competitive and expensive.
However, the local traffic is relatively low, and less competitive keywords will not have enough searches per month.
I need you to look at my case and tell me what is possible and what traffic/conversion rate I can realistically expect with a given budget.
