C# class to create a PDF (new)

I have 1 simple form in Word format. I need a class to generate the same form in PDF using c#. It's mandatory to use the itextSharpLibrary since we already are using it in the project. I give you the forms in Word and the data structure as some simple C# data classess. You can find this material in the attachment. Inside the word document you'll find some notes in red. Two parts of the forms are "1...n", this means they could be repeated n times. In the data structure you'll find I used a list to describe them. Please contact me to ask any question, but I expect most of the things should be clear just reading the enclosed classes, and you are an expert of C# and already used iTextSharpLibrary or similar. Please comment your code and write it VERY CLEAR. The class should work with Visual Studio 2008 and .Net 3.5
