Access 2010 Web Database Design

This project is for a simple Microsoft Access (2010) web database to be used on a Sharepoint server. The intended purpose of the web database will be to serve as a data repository for basic client information as well as dated fitness measurements. For example, the database will have clients with fitness measurements for each client from particular dates.
The database will consist of 3-4 tables (Clients, ClientMeasurements, Comments, etc). There will need to be a couple of queries, forms and reports required as well. Furthermore, the database will also need a navigation form from which we can:
1) add new clients
2) edit current clients
3) add client measurements
4) remove clients
The database needs to be in Microsoft Access 2010 with web compatibility since it will be hosted on a Sharepoint server.
The database could be modeled after the web database "Contacts" template that is provided with Access 2010 however, with some modifications to the table fields.
