C# Application Modifications - Display Database Table in DataTable Format

Currently visitors enter their information in a website form which is transferred to a database at any time of day. Previously a small desktop application was created to produce a pop-up on the desktop computer than would display the information entered. However, once the popup was exited that information would no longer be accessible to the desktop computer. We are looking to extend the functionality of this software by allowing the user to see the full history of the sign-ins. This will require a few slight changes to the existing pop-up mechanism and a simple datatable layout created from the database table. I have attached a detailed requirements document and a mockup of the UI I am looking for. This should tell you that it's a quick job with everything laid out without the normal back and forth questions. In your proposal please demonstrate some sort of knowledge of the document or how you plan to execute it. 50% of the total payment will be released after testing to verify it is all working and 50% of the total will be released after the source code has been delivered (both application source and install project source).
