C# MVC 4 setup Membership Provider for MSSQL 2005

You know how if you create a new C# MVC4 Internet Application in VS 2012 how the SimpleMembershipProvider gets you up and running with users/authentication right away? Me too, and it is great EXCEPT ... I have a legacy user database from .NET v2.0 and I need to continue to use it. So I need to swap the default SimpleMembershipProvider to something that will work. But simply copying and pasting this won’t work on its own. Either it needs to be formatted differently or there are code changes that I don’t understand. So I need help!

My database is MSSQL 2005 and its schema is the same if you initialized a website with aspnet_regsql.exe.

The deliverable would be a simple hello world site that allows for a few users to login and logout. That is it. No design. no styling. I just need this configuration figured out so I can move on with porting my existing site.

I’ve attached an empty DB, but you can also create your own with aspnet_regsql if you’d prefer.
