The actual dimensions of the attached paper is 28.75cm by 20.4cm (slightly smaller than A4). I would like a form-fill-able pdf created so that I can print to the physical form DIRECTLY after entering the data onto the form. Fields 1-9 are forgiving and should have 0.5 cm left between the to-be-filled text and the lines. Field 10 has four blanks to fill that are in-between text, attempt 0.2cm blank space around the to-be-filled text. Same for field 11. The first blank for field 11 is tight and will forgive if the text slightly overlaps the dotted line. Good luck. I have another near 20 forms of such nature to be created and will assign them to you if this one goes well.
