COde Re-write

Take a php script and re-write it to aspx with small changes. ## Deliverables the php script that is listed on this bid: Take it and rewrite it using aspx and make several small changes. I want the form reduced to only the cell phone number. I want to be able to be access via a clickable image or link on a database results page. When the user clicks the image or link, a small window should pop up to allow them to enter their cell phone number. The script also pulls several result fields values from the database results page the user clicks the link or image on. (ex. <%=rs("sample_field")%>. That simple. That is all.
