Basic PHP form to feed MYSQL database + search based on entries - repost

Hello coders,
I need a basic PHP form with 8-10 parameters (text, tick box and drop-down) to feed all the required info in a new MYSQL database.
Then a 2nd form to search those entries by 4-5 of the initial parameters.
The forms and the results would be included in a wordpress standard post/page.
The CSS needs to be minimal, just black and white, nothing fancy.
At this moment I don't need and admin page, but that is an option for future in a new extended project.
As it requires only basic PHP/MYSQL, the budget is up to 50$, so please bid only if interested.
