Create PHP FPDF Contract From MS Word Template

You will create a replica of a contract document using PHP and an open source PDF class. (probably FPDF). The contract is 9 pages. There are approximately 40 lines per page which include some images, checkboxes and places to fill in information etc. The formatting of this new contract will need to match an existing blank contract which I will provide. The fill-able fields on the PHP code will simply need basic placeholder variables. I will take care of the data that is being passed to the form along with all database activity. You must simply declare the array to avoid errors when some or all values are empty. Your job will be complete, when: 1. The php code is parsed and the output is PDF and looks just like all 9 pages of the existing blank agreement. 2. Some or all variables of the array are defined and the corresponding fill-able fields are populated with that data, looking like the blank form was filled out. Please do not bid on this if you are not well versed in PHP-FPDF or similar. You must be able to communicate effectively & easily in english. I am open to hourly or project based bids. Time frame to start this project is immediate. Time frame to complete the project is < 1 week.
