I need a capture page for a website. Simple design and intuitive. I have the domain name, that is it. I know what I want, but need help. The capture page is simple. It will ask for a person's name, phone number, able to accept text message, physical address, email address, cross street, and closest city. I would like this information to be captured on a Google Doc. I would also like to be notified via text message when someone submits informatio.. I also want there to be an option to purchase the service the website is offering. This will lead to more work as the business grows.
