Create Custom Infusionsoft Order Form/Sales Page (MUST BE FLUENT IN ENGLISH)

I need someone to create an order page very similar to this page - [login to view URL] , but the page will need to be updated so the order form works with my Infusionsoft order form (legacy), as well as some other adjustments to the page. I don't own the page at the link above, so you'll have to somehow clone it, or whatever you need to do to use it as the basis for my page.
