I need a simple PHP script with the following functionality:
1) User inputs a total minute count of audio or video file(s)
2) PHP script calculates the cost based on Service selected and rate per minute (Excel rate table provided)
3) The total projected quote cost is returned on screen
4) Report capability that includes date/time, services selected and quote amount.
