What we want to create is a a few layer online form, that requires an email and password to enter.
- We want the user to fill out some info through a 3 step process
- Step 2 will require the user to sign the form with a online consent signature that is legal.
- Step 3 we would like the form to request the either scan their ID via the camera on their computer, or take a picture with their phone and upload the file in the form.
Once they press send, it will email the completed form to a gmail secure email, and record their IP address for extra confirmation.
We want it Mobile friendly and the ability to scan and sign via an app as well.
