I need a contact style form created. There is nothing especially special about the form, However the form is fairly long as it is an application to apply for financing. I need all of the usual checks on the form (email address is correct, correct format for entering name, for numerical fields make sure text is numerical, etc.) It will need checkboxes, etc. just as in the pdf. The disclaimer type text I will do -- I just need the form part created. When the user has filled out the form, on submit, it will send the data to an email address. I need the form in an html file and the backend page is a .php. I will fill out server and all such personal information on the php page, just fill in with some dummy text for now. The format of the form can be all in-line. For example, the form could look like this:
Application Date: [field here to fill in]
Seller Name: [ ]
Applicant Section....
(Then Below that section)
Co-Application Section
I will test form before payment is made. Please use descriptive variables. You can use dreamweaver or any other IDE, so long as the form works. If you have any questions, please let me know.

I can make the form with the specified requirements. Will code data validation and error highlighting using jquery. PHP will handle validation and return results to the from via json if there are validation errors. If no errors there will be hook where you can now do anything you require with the data.
Since the form is long, I can break it down into sections which must undergo validation before proceeding to the next.
For things like dates, we can use UI calendar widgets provided by the Jquery UI library.
Code will be document and I will purely use functions.
