Custom Code a Page in WordPress w/Database

I would like this to be able to be put into the WordPress page located at [login to view URL] I need to be able to edit the other elements of the page so we need to figure out a way to get the elements I'm requesting into that page. The following text will appear first on the page: "Track the Happiness if you want to know where your Happiness Coin has been, or what happiness it will spread once you share it, all you need to do is enter your zip code, good deed done to you, good deed you did (or plan to do) and serial number of your Happiness Coin. If you register, you will be notified by email if your coin has been entered again into the system and what good deeds were done thanks to you passing along the coin (this is the best part)! Once you submit the information below, you will be able to view the good deed history of your coin. Please enter the following information:" Then we need to collect the following information: Coin serial number (text field) Your current zip code (text field) What good deed did the person who gave you the coin do? (text area) What good deed did you do (or plan to do) when you pass on the coin? (text area) The following text should be displayed below the fields: "You can enter any USA Zip or Canadian Postal Code. If you do not know your zip code, click here" A similar popup that is on [login to view URL] should pop up and tell them the zip codes that are in their area based on their zip code. Below that, please display this text to users: "Please notify me when the next person records the coin's location and good deeds (highly recommended…this is the best part)!" Include a slide box with Yes or No. If the user selects "YES"… the following should automatically appear. Your email address will remain private and will not be shared with anyone. Ask for the user's full name (text), email address (text), and password (text). A database will need to be created to store all of the information collected. The user's name and email address will also need to be sent off to [login to view URL] (I will provide the form code). When a user enters a serial number, it should check the database to see if anyone has entered that serial number before. If someone or a group of people have entered that serial number, those users should be emailed with the location of the coin, the serial number, the good deed did the person who gave you the coin did, and the good deed the person did (or plans to do) when they pass on the coin. Now, the ZIP code will be in the database but when we display the location to users via email or on the site itself (in the sidebars), it should display the city and state of that zip code, not the zip code itself. For example, if the user enters in the zip code of 23113 what should actually display to users should be Richmond, VA. On the left side bar I need the following stats to display: # of Coins in Circulation (start with 5,124 and increase by a few coins every hour) # of Good Deeds Done (start with 23,341 and increase by a few good deeds every hour) On the left side bar, I also need to display a Leaderboard with the sites top 5 people who have done the most good deeds. I also need another Leaderboard displaying the top 5 people who have put the most coins into circulation (the first to enter a coin into the system) On the right side bar, the latest good deeds entered should scroll (both good deed fields) along with the name and location. There should also be a login box (user name and password). The user name should be their email address.
