Custom Static Form

We have 2 separate Landing Pages we have created on our DEV server. We need to have a simple form setup to submit the details via an e-mail and also have it save the information submitted in a .csv file format right onto the server that can be downloaded and imported into a separate database. We can't have these submitted into a database on the server, which is why we need a separate way to save the information like a .csv file. If you have a better easy to use solutions that these files pages can be uploaded to the live server without any custom changes. The Landing Pages, as they appear now need to stay the same way. The only thing we are asking for is the form fill funtionality. This will need to still validate all of the Fields and detect if Address/Country are not within USA or Canada and not require State/Province and Zip/Postal if it is outside.
