Advanced JQuery Form

I am looking to have a form made with: Prepopulated country/state restricted to US, CA, UK When the country is selected the state drop down should change to show only states within that country. If the country is US then the masking script should be used to mask the input like this: (___) ___-____ If the country is not US then it should use a numeric only text box instead. The jquery plugins that should be used [login to view URL] and [login to view URL]
