I would deliver the new design as an image - together with the complete old website (HTML, JavaScript, PHP, MySQL). The old project is disordered - NO seperation of HTML, JavaScript and PHP, ...
There are about 60 PHP files (about 700kB - including HTML, PHP, ...) - with about 10.000 lines of code - BUT you don't need to change so much - NO PHP, ... should be changed - only HTML and images, fonts, backgrounds, ...
You should have experiences with PHP and MySQL - also you don't need to change anything there.
The design image must be cutted to fit the basic layout.
For me it would be important that I can talk to the developer by email, skype, ... directly in English or German and you provide a test installation on your servers, where I can test all results.
