Create Simple Magento Plugins

I have a website for membership e-commerce based on magento. ! sell a membership-discount card. So if you have our card, you will get discount in a certain hotels. The flow is like this : 1. Customer purchase the card. 2. I receive payment and send the card to the customer. 3. Customer receive the card, and inside the card, there is instruction on how to activate the card. 4. Customer activate the card by visiting the link given, input First and Last Name, Card Number, etc. 5. After that, customer will receive email stated that the card has been activated. 6. When customer go to a hotel, customer will show the card, and the hotel staff will go to my webpage, input the card number or email address or last name to verify the card. So I already have step 1-5. I need step 6. All the customer attribute already inside the magento. I need a plugins to create a page that show : Card Number, First Name, Last Name and Expiry Date After hotel staf input : Card Number or Last Name or Email Address. Expected output is plugins. FTP access in testing environment will be given. Thank you
