Asterisk AMI Code

We are using Asterisk 11.5.1 and FreePBX 2.11. I need a script in PHP that will receive 2 numbers, and this script should connect to Asterisk Server using AMI, originate a call to both numbers (will be always external numbers) and bridge them. Both calls should be dialed using the Outbound Routes defined on FreePBX. This call should be registered on CDR, with the defined account code. The script should handle expections and report to user.
