Hi there,
I need some small customizations done for the following script:

I need to add fields to the forms on the "Create an Account" and "Create a ticket" pages which will result in changes elsewhere in the script.
"Departments" must be changed to "Issues" in all instances as well.
Will provide more detail at a later stage.
