A small site with even smaller functionalities in PHP

I need a very small site created with simple functionalities using PHP. Please refer to this link and follow the steps below: [login to view URL] 1. Page layout, color combination and fonts will be like this page. 2. As you can see, left side will contain menus and main contents will be in the middle. 3. List of menus and submenus will be same for all pages. I will provide the list. There will be one page for every submenu. 4. Administrator will fill up a form every day from every page. Typical field of the form will be, but not limited to, Process Title, Description, Author, etc. 5. Normal user, once logged in, will see the list of process title those were entered so far. Clicking on the title will show her/him all details those were entered by the administrator in step 4. Please note that the codes should be well structured, neat and clean, and properly commented.
