Add features to new website

We need extra functionality added to our website that will enable users to track their medication history. Our website needs the ability to register a user account and login to access their own portal where they can record information about the medication they have been taking. The website will need to capture the name of the medication, type of medication (ie paid killers, blood pressure medication etc), who prescribed it, why you started taking it, the date you started the medication and the date you stopped, also the ability to record notes about your experiences on the medication. This information will then need to be listed in their portal so they can access it at any time. This project needs to be completed by the 6th of April.
