ACF13 Create

I require a very straight forward form creating in PHP.? This form will consist of? a dropdown list and a submit button. The drop down list will look up its values from a MYSQL table.? The table will consist of 5 fields, pkID, CampName, FromDate, ToDate, cost.? The dropdown list will show the following “CampType??, “CampLocation?? + “FromDate?? ??" “ToDate??, “Cost??. The user will select from the drop down list, and click the submit button.? With the submit button clicked, the form must create a PDF documents in a new window and insert the CampType, Location, FromDate, ToDate and Cost into specified fields. I will supply the document in a word format, which will be required to be reproduced as PDF documents. I require this small application to run on a Linux server with PDFlib. I require this application to run in an <iframe> on an existing website.? I require this project to be completed by the 20th November.
