Create a ranking system and comment system

create a ranking website that allow me to attach / upload 12 images jpg/gif to a webpage. Users can then come and rank the images in order of preference. this order is emailed to an address and a txt file is update that keeps track of all the total score of each ranking so that I can figure out which one got the highest ranking. each User will be able to make a comment on each item, that will be emailed and attached to the txt file . but comment will not be available for other user to see. I should add a total of four question next to each item being ranked and the user will be able to pick radio buttons A, B, C, D of which I will define. Another question will be a blank which user can input a value. result will be stored in a txt file and also emailed to me at any email i enter.
