Custom Online Survey

We want to create a survey online so that when people respond it will produce a customized assessment in real time based on the data they submit. This is how it would work. A person would answer a series of 5 questions and then hit [login to view URL], based on the answers we would give the person an assessment that is tailored to their particular responses. If their answer to question #1 is an 8 out of 10 ranking, for example, they would get an assessment that is specific to their response. If their answer is a 6 out of 10 ranking, they would get a different assessment. We would have a series of if-then statements based on the responses. The person filling out the survey would immediately get a report that is customized based on how they answered each question. We would also want all the data stored in a database for us to access from time to time so we can analyzed it and create benchmarking reports later.
