Create new "Become a Member" & Sales Rep pages in PHP site

Project for BryceP only. Create new "Become a Member" section in existing site that gives site users a way to become a member and choose to pay for a rewards card at the same time. Also the issuing of unique member ID. Create a new Sales Rep area where our sales team can keep track of businesses they have signed up, including a % of their generated income. % is based on an amount we specify.
