Add image/file links at the bottom of a PHP page

I’ve got a PHP page I’ve been working on and I need a section at the bottom that will allow users to upload image or just a general file. It can be very generic/basic. I’m thinking 3 static textboxes and “Browse…” buttons for the user to choose the file from their PC. After they select the file, I just need that textbox (or just the area if you wanna leave the textbox there) to show that file (as hyperlink if you want) in that area. Then, they have the ability to add 2 more files if they’d like or delete that file. You do not need to work about the size of the file as there doesn’t need to be any error checking for this section whatsoever. Just need it to work w/ smaller files. So, basically, I need a section in between the image and comments section (or above the report it now button if it’s easier) that allows users to add 3 files. Also of note, is that I need this info saved in a MySQL DB table and for an email to go to a hard-coded email address (I will provide it) w/ the attachments (I will update the message in the body message when you get the page back to me). Also, I don’t care about the schema of the DB table. I just need you to tell me what the DB table name is, so I can create it on my local DB for testing. Prolly be easiest if you just provide me w/ script to create the table. See the attached Word doc for a screenshot of the [login to view URL] where the section would need to go.
