I need a form like the one from picture 1 to submit to a "database" like the one in picture 2. At the same time, once picture 1 form is submitted, I would like a process to run (no clue which language) in order to log into the portal in picture 3 and then push the picture 1 form data to the picture 4 form and press submit. Screen scraping will likely be necessary. My price might be too low so let me know. I will handle all CSS and formatting. The work would just be getting the functions to execute.
