This is a simple project for a skilled PHP / Joomla developer.
In my experience, I estimate the single PHP file and form design to take about 3-4 hours maximum.
The successful bidder will be rewarded with ample additional work as another 40+ pages need to be developed with much greater complexity. Futhermore, there will be new code releases required throughout next year and site maintenance. We can discuss maintenance contracts later.
