Customer database for a Tanning Salon Chain

A database is needed to store basic customer information. We have approximately 2000 customers per salon. I need contact details to be stored, a picture or scan of Identification ([login to view URL] or drivers license) to be added to that customers homepage, maybe through a scanner or a webcam. Staff need to be able to create a new customer entry which will include a basic ticklist medical check (tick yes or no to contra indications for sunbeds).
Staff then need to enter the session time for today, and the program will then transfer this to the customers history. Basic flags or recommendations need to be brought up by the program if that 'session time' is out of what I will define as normal parameters. A warning message needs to be acknowledged by the staff who either agree with the original session time, or enter a new lower session time. The system needs to be as simple as possible to allow staff quick access to records.
Lastly, a reporting function needs to be a part of the program. That is, staff click end of day report and cumulative session times, average session (total session time divided by total numbers of customers) and total days takings. This can then be printed off.
