Buggy Access - Cannot scroll through records

Please see attached access database. In the scheduledSurgery form, you cannot scroll through the records. I do not think the updates on this form are working properly either. Please fix!! The firstname, lastname, surgeon, and patientID are pulled from the patientInfo table. These fields should NOT be updated from this form. The other fields on this form are on the scheduledSurgery table. The firstname/lastname field is working like this: When user opens up this form, and goes to the last record, he will have to press ADD to add a new record. The screen will clear except for the firstname/lastname fields. The lastname is a drop down menu. When lastname is selected from dropdown list, the firstname dropdown menus changes. If there is multiple patients with the same lastname, the user will have several choices in the firstname dropdown box. After user selects lastname/firstname combo, the patientID and the surgeon should be pulled from the patientInfo table. THIS IS NOT WORKING!! Also when user tries to scroll through the scheduledSurgery table, that does not seem to be working either. Buyer will receive exclusive and complete copyrights to all work purchased.
