Complex Web Site Client Web Data Entry Form

Develop a web site that allows clients to enter data into a web form(s), upload documents and communicate with client support. The design work for this site is done. Work closely with me to complete the remaining client data entry web interface. Technologies: HTML, HTML5, CSS, JavaScript, NoSQL (Mongodb or CouchDB). In short, there are 8 pages in this project. login, passcode recovery, main page, application data entry, document upload, message dialog, and a few very simple information pages. However, the main page has several lookup elements that need to be displayed. This is phase 2 of a 5 phase project. The design phase is complete so you'll have these blue prints to work from. However, you must allow me (and expert in JavaScript/HTML5) to monitor the web development at bi-weekly agile/scrum meetings via Skype. Project Milestones and Payments are bi-weekly. So, those who can't deal with this shouldn't apply. This will be an enterprise application that will be deployed worldwide. If the project goes well--I'll offer the second phase and longer-term support contract to the vendor (freelancer selected). So, if your an entrepreneur looking for follow-on work (Phase 3, 4 and 5), this is a great project. For the right vendor/freelancer I would offer an ownership stake in the final application--between Phase 4 and 5.
