Design a PHP Proxy Website

I've looked around but cannot find one premade one that I like. It's an odd request but would be awesome for you if you have an old proxy site laying around. I just want the whole package: *Domain Name and Hosting Only with a hosting plan that allows php proxies. Please include the first monthly hosting price of the company you would use in your messege. (OR, you can just make the site and show me how to get it hosted somewhere) *Glype Installed I want this script because of the abundence of themes and ease of use. *Old sites are fine, you can send me a link in your message. If you have one laying around that you don't want anymore this is really easy money!
